3. Enrollment Inquiry Form and Registration Form — Not a Binding Contract
Enrollment Inquiry Form. Our enrollment inquiry form (available at /enroll) allows you to provide your contact information and ask questions about our programs. By submitting the form, you authorize us to contact you by phone, email, or other means to discuss your inquiry. You are not obligated to enroll in any program, and we are not obligated to accept your inquiry.
Sensitive Information in the Enrollment Inquiry Form. Our enrollment inquiry form collects certain sensitive information to help us understand your situation and match you with the right program. This includes: your readiness for a DOT medical examination, your readiness to take a pre-employment drug test (5-panel DOT urine screen), and your intended funding source (such as self-pay, employer sponsorship, or a workforce grant program). This information is collected solely to facilitate your enrollment process. This information is not used for any purpose other than helping our staff contact you and prepare for your enrollment conversation.
Registration Form / Enrollment Agreement. The Registration Form (also referred to as the Enrollment Agreement) is a separate document from the enrollment inquiry form. If you choose to proceed with enrollment, you will enter into this separate Enrollment Agreement that will govern the terms of your training, payment, and other details specific to your program. All applicable federal and state laws and regulations will apply to this final binding contract as explained in our Course Catalog.
When the Enrollment Agreement Becomes Binding. The Enrollment Agreement becomes binding only upon receipt of the signed agreement, payment of the initial deposit or first payment, and confirmation of your start date by Taylor Made TDS, Inc.
